iCardiac Rehabilitation is an outpatient program that focuses on improving your cardiovascular health. If you have been diagnosed with coronary artery disease or experienced a heart attack, angioplasty or open-heart surgery, you may be a candidate for Cardiac Rehabilitation.
Heart health is about more than just your heart. Blood pressure, cholesterol, family history and other factors all play important roles in keeping your heart strong. We tailor your rehab program to meet your individual needs, combining supervised exercise training with dietary and health education in a supportive environment. Cardiac rehab could help you strengthen your heart and reduce your risk of further heart problems so that you may return to your active lifestyle.
You do not need to go through cardiac rehab at the same hospital where you received cardiac services. A patient may receive cardiac treatment elsewhere, but we can provide the follow-up care close to home.

You may qualify for cardiac rehab if you've experienced the following:
You can be referred to the cardiac rehab program by your primary care doctor, cardiologist, surgeon or other healthcare professional.
To get referred to the program, you must have one of the following diagnosis. Then you must visit with your primary care doctor, cardiologist, surgeon or other healthcare professional to provide a referral.
