Medical Records

Castleview Hospital medical records department is available to help you gain access to your records. 

Medical record requests may be made by calling the department at 435.637.4800 ext. 4200.  The medical record request form will need to be notarized prior to being processed. 

If you are requesting all of your records, we provide 25 pages for free, then it is 50 cents per page.
